A new study published in Catheterization and Cardiovascular Interventions evaluated the diagnostic performance of 2DμQFR (Murray lawbased quantitative flow ratio) against wirebased FFR, comparing it with 3DμQFR and vFFR.
The VERMONT μQFR substudy included 205 intermediate coronary lesions (5070% stenosis) from 209 patients. 2DμQFR was computed retrospectively from a single angiographic projection using AIassisted vessel segmentation.
Key Findings:
📌 Excellent diagnostic performance — 2DμQFR achieved an AUC of 0.95 (95% CI 0.93–0.98) with 93% sensitivity and 92% specificity.
📌 Comparable to 3DμQFR, superior specificity to vFFR — 3DμQFR showed an AUC of 0.94; vFFR showed an AUC of 0.92. 2DμQFR demonstrated higher specificity (92% vs. 79%) than vFFR.
⏱️ Time efficiency: 2DμQFR analysis took only 1.8 ± 0.7 min – 15 minutes faster than wirebased FFR (16.9 ± 5.4 min) and faster than vFFR.
🔍 3DμQFR evaluation – When a second projection was added, 3DμQFR achieved an AUC of 0.94, sensitivity 89%, specificity 90%, and diagnostic accuracy 90%. These figures are comparable to 2DμQFR, suggesting that the additional geometric information from a second view did not confer a clear diagnostic advantage over the singleprojection approach in this dataset.
AI advantage: Manual contour adjustment was needed in only 32% of 2DμQFR analyses, versus 77% for vFFR, reflecting the benefit of AIassisted segmentation.
💡 Clinical implications:
Singleprojection analysis enables physiological assessment even when optimal dualview imaging is not feasible.
2DμQFR was successful in 85% of lesions that were excluded from vFFR analysis due to image quality limitations.
Excellent interobserver reproducibility (ICC 0.97).
